The most crucial step in rendering your treatment and providing you with an exceptional experience is gathering information on your present and past. Your cooperation to complete our new patient intake forms thoroughly, will help to ensure success on our dental journey. Getting to know you and your dental history is our first priority, so that we know how to best serve you moving forward. Your dental health is also strongly linked to your overall health, so the more information you provide to our dentists, not only will make your results more predictable, but you will be happier and healthier! Please make sure to list all your current medications, as they can affect your dental treatment as well. Also, if you are currently undergoing medical care, please provide the names of your medical providers. Most Importantly, communication is the key to rendering your dental care.

Please download, complete, and email the completed patient intake forms back to us, or simply bring them with you to your first scheduled appointment. They will be reviewed in your first meeting with Drs. Rinando and Jusino. Please also bring a copy of your current dental insurance card, as well as your drivers license. Doing so will enable us to send your insurance claims electronically on the same day as your treatment so that your reimbursement will be expedited. If you have any recent x-rays or dental records please either bring them with you to your first appointment or you can always email them to us ahead of time for review prior to your consultation. Financial Policy

Treatment will only be rendered after you are aware of the costs, and we never pressure. For this reason, we always offer FREE introductory consultations enabling you to meet our doctors and tour our office. As a courtesy to our patients, we send all insurance claim, radiographs, and treatment documentation to your PPO dental insurance provider. However, it is the 6th Sense Dental financial policy that we collect payment in full at the time your treatment is rendered. We believe our treatment recommendations and philosophies should not be dictated by a third party, so we are not an “in-network” provider, and ask your PPO insurance company reimburse you directly.

If your insurance is HMO/DMO then you must go to a dentist on the list of providers your insurance will provide in order to have coverage. We are an advocate, but are not in control of your dental insurance or what they chose to reimburse you. We do offer third party financing for treatment through Care Credit, Greensky Bio, and Lending Club. Also, we offer pay in full case fee discounts to help affordability in giving you the smile you’ve always dreamed of. It’s our goal for you to be able to receive your treatment based on your desired results, and will do our best to help make it affordable for you. We accept most major credit cards and checks for payment at time of service. We will happily send or forward you all receipts for treatment and payment to assist with your own tax documentation.

What can I expect at my first appointment?

What can I expect at my first appointment? Your initial appointment will include a tour of our office and meet and greet with Dr. Victoria and her team. We either offer free consultations, or the doctor can also perform a comprehensive examination of your oral health to discuss any concerns and determine a treatment plan to meet your needs and goals.

What are your office hours?

We offer convenient hours to accommodate your schedule. Office hours: Monday : 830-430 Tuesday : 830-430 Wednesday : 830-430 Thursday : 830-430 Friday : pre-arranged only  *Private client hours are available for large cosmetic cases outside normal business hours.

Do you offer emergency hours?
Yes, we understand that dental emergencies don’t always happen during regular office hours. We do our best to respond to your calls 24/7.  Existing patients receive their dentist’s direct cell phone number.
Does your office submit insurance for patients?

As a courtesy to our patients, we electronically submit your claim and records to any PPO dental insurance plan for you.  If you have an HMO/DMO you must go to to the list of dentists your insurance provides you.

Do you offer free consultations?

Yes, second opinion consultations are free and includes an office tour, plus a chance to meet and greet our team of dentists and periodontal surgeons to discuss the best dental treatment options for your individual needs.

What types of payment do you accept?

Do you offer sedation?
We offer oral sedation, nitrous oxide, and IV conscious sedation so that your dental experience is as calm and serene as possible.

Routine dental exams and regular cleanings are the best defense in preventing the incidence of higher-cost treatments such as periodontal surgery, root canals, extractions and fillings. For optimal dental health most people should have their teeth cleaned every 3-12 months depending on your individual oral health.
