These provided patient resources are here to help make your first visit as pleasant as possible! The most crucial step in rendering your treatment and providing you with an exceptional experience is gathering information on your present and past. Your cooperation to complete our new patient resource intake forms thoroughly, will help to ensure success on our dental journey.
Please download, complete, and email the completed patient resource forms back to us, or simply bring them with you to your first scheduled appointment. Please also bring a copy of your current dental insurance card, as well as your drivers license. Doing so will enable us to send your insurance claims electronically on the same day as your treatment so that your reimbursement will be expedited. Please make sure to list all your current medications, as they can affect your dental treatment as well. Also, if you are currently undergoing medical care, please provide the names of your medical providers. Most Importantly, communication is the key to rendering your dental care.
If you have any recent x-rays or dental records please email them to us ahead of time for review prior to your consultation. We appreciate you taking the first step in making us your new dental home. Rest assured we will care for you as we would our loved ones. We absolutely love what we do!
Financial Policy
Treatment will only be rendered after you are aware of the costs, and we never pressure. For this reason, we always offer FREE introductory and second opinion consultations, enabling you to meet our doctor, and tour our office. As a courtesy to our patients, we send all insurance claim, radiographs, and treatment documentation to your PPO dental insurance provider. However, it is the 6th Sense Dental financial policy that we collect payment in full at the time your treatment is rendered. We believe our treatment recommendations and philosophies should not be dictated by a third party, so we are not an “in-network” provider, and ask your PPO insurance company reimburse you directly.
If your insurance is HMO/DMO then you must go to a dentist on the list of providers your insurance will provide in order to have coverage. We are an advocate, but are not in control of your dental insurance or what they chose to reimburse you. We do offer third party financing for treatment through Care Credit. Also, we offer pay in full case fee discounts for large cases in order to help affordability in giving you the smile you’ve always dreamed of. It’s our goal for you to be able to receive your treatment based on your desired results, and will do our best to help make it affordable for you. We accept most major credit cards and checks for payment at time of service. We will happily send or forward you all receipts for treatment and payment to assist with your own tax documentation.
